Before submitting a claim, it is important that you read and understand your builder’s warranty. All claims submitted on this form must first be inspected and reviewed by a PHWS representative. No items listed on this form will be scheduled for repair and neither PHWS nor your home builder will approve the warranty status until the item is inspected by a PHWS representative.
By filling out this form you should expect to be contacted by the PHWS office and your builder’s customer care / warranty department within 24 hours to schedule an appointment for your noted items.
